How to obtain a transcript:

1) Your request must be in writing with an original signature and a payment of $5.00 per transcript.

2) Information needed: Year of graduation, name at the time of attendance, social security number, current address, and the address where the transcript should be sent  to.

3) Do not contact the Registrar's Office in Evanston, if you do they will forward your request and it will delay the issuance of your transcript.
